Initial Consultation (new patient visit)

Service Description

Our first session will include gathering all of your health information and getting to know you as a whole person. We will address your health concerns and goals. I will also get a thorough medical and social history, as well as ask about any past or current medications/supplements. I will perform any necessary physical exams and possibly order any labs or imaging if necessary. You will leave with a clear treatment guideline and plan for follow-up. Please upload any pertinent labs or medical information to the 'documents' section in your CHARM patient portal.
